• Develop new referral partnerships and strengthen existing healthcare and community relationships.
• Serve as a trusted resource and collaborate with internal teams to support clients and referral partners.
• Represent Home Helpers at community events, healthcare meetings, presentations, and networking opportunities.
• Manage and grow the territory through consistent outreach, relationship development, and follow-up.
We encourage applicants with experience in social work, human services, healthcare case management, hospital or skilled nursing discharge planning, care coordination, patient navigation, senior services, community health, healthcare outreach, or business development. Preferred qualifications include experience in healthcare, social services, case management, community outreach, or relationship-based sales. Strong communication, relationship-building, organization, and time-management skills are also important. A genuine passion for helping seniors and individuals who need additional support is required. Self-motivation, ability to work independently, and willingness to travel throughout the territory are also necessary.
Competitive base salary of $50,000-$60,000 annually and monthly performance bonus opportunity. Full benefits package and company car provided. Flexible schedule and an opportunity to make a meaningful difference in the community.
